Local Action Teams

Local Action Teams (LATs) are a key component in the structure of the Child and Youth Mental Health and Substance Use (CYMHSU) Collaborative. They provide the foundation for improving timely access to support and services for local children, youth and families experiencing mental health and substance use challenges.

Teams are comprised of a diverse cross-section of mental health and substance-use service providers, stakeholders, and youth and families from the local community.

How it works

Each Local Action team commits to tackling one or two key objectives within a particular timeframe to address system barriers in their community. A Collaborative Coach supports the team to help them successfully achieve their goals and measure their outcomes.

Polypharmacy Initiative


Abbotsford family physicians hand residential care teams have been engaged in Polypharmacy Risk Reduction and have been provided with tools and resources to support meaningful medication reviews within the context of the GPSC Residential Care Initiative.

Abbotsford has Polypharmacy Risk Reduction physician mentors who are skilled in mentoring their physician colleagues in meaningful medication reviews in residential care.

Partners in Care Initiative


Transgender Patient Pathway

Care providers in the Abbotsford-Mission area are seekinga clear and consistent process for transgender specific health care services. The existing services are not widely known by the transgender patients and the health care community.

Cervical Cancer

Bring GPs, specialists, Fraser Health, BC Cancer Agency, key community organization and patients together to develop culturally appropriate communication and clinical pathways for cervical cancer screening. 

Strengthening Medical Imaging (Spread)

Improve timeliness and appropriateness of medical imaging exams for Abbotsford patients through improving the referral process, identification of level of urgency, education about appropriateness, and improving communication between medical imaging and GPs.

Develop alternatives to Maternity Care - Complete

Project aims to bring GPs, specialists, Fraser Health, community organizations, and patients together to address gaps in communication and awareness around maternity care and services in Abbotsford and Mission.
