Shared Measures Orientation Session #2

To support the Shared Care Committee’s goal of advancing the Triple Aim, the Shared Care Committee has adopted a system of shared measurement for all funded projects.
Following the first session on January 20th where the focus is how to integrate shared measures into project and evaluation planning, the second session will focus on data collection and reporting.
