Update: Shared Care Project Funding Program re-launches

We are pleased to officially announce the launch of the updated Shared Care Project Funding Program, formerly Partners in Care/Transitions in Care.
The Shared Care Committee (SCC) began its work in 2010, supporting improvements through collaboration between family physicians and specialists. The funding portfolio continues to evolve as SCC sets its future direction in alignment with priorities established by the Physician Services Committee.
What happened
A significant change occurred in early 2026, with new application intakes paused for several months to allow for an extensive review and refresh of the program. The revised framework and guidelines reflect the updates implemented through that process. The approach to the new funding framework was approved by the SCC, but was built by a robust working group (13 members, including physicians, quality improvement specialists, Ministry of Health and health authority representatives, and Doctors of BC staff members), and informed by more than 200 project team members to guide the direction and understand the impact.
In Quality Improvement, we see change as an opportunity to test and refine. As with any change, we will monitor, evaluate, and respond as needed, anticipating questions as we navigate the rollout’s changes. Updated documents
In an effort to launch these new documents with ample time before the first application deadline (August 15), we have prioritized releasing the updated guidelines, application forms, and immediate supporting documents. You may notice that some documents, such as final report templates, may still require updates to align with our new pathway language and varying funding stream expectations. These will be prioritized immediately after this launch and updated throughout the summer. Thank you for your patience during this transition process.
