Shared Care Project Funding

Overview

Shared Care project initiatives focus on family physician and specialist co-led projects to improve coordination of care for patients among providers and between care settings. Project activities are most commonly focused on improving at the community level, with strong potential for regional or provincial spread.

In February 2026, project intake was paused to allow for a process review. The funding program updates have been finalized and were communicated in a letter from the co-chairs on May 8, 2026.
