Fraser Northwest Division's Advance Care Planning (ACP) initiative was launched in April 2013 with the support of the Shared Care Committee and the leadership of Dr. Joelle Bradely, Royal Columbian hospitalist and former board member.
Now into its second year, the ACP has four hard working committees and will be hosting its first Advance Care Planning Fair on Saturday, April 11, 2015!
