Partners in Care/Transitions in Care

 

In February 2026, EOI intake was paused to allow for a process review. The funding program updates have been finalized and were communicated in a letter from the co-chairs on May 8, 2026. Note: Until further notice, funding for cancer care-related projects through the Partners in Care/Transitions in Care initiative will be paused. Shared Care has put out a Call for Proposals focused on cancer care and has allocated funding for both expressions of interest and full proposals resulting from this process. The SCC would like to direct any cancer care-related projects through the Call for Proposals process in order to best assess, connect, align, and make decisions on the proposals to maximize coordination and support from health system partners. 

Overview

Partners in Care (PiC) and Transitions in Care (TiC) initiatives focus on family physician and specialist co-led projects to improve coordination of care for patients among providers (Partners in Care), and between care settings (Transitions in Care). PiC and TiC activities are most commonly focused on making improvements at the community level, with strong potential for spread regionally or provincially.
