Networks
Shared Care's approach to supporting the spread of successful work, is to build networks linking communities with an interest in improving care for specific priority populations.
These networks are enabling providers to support one another in sharing resources, tools and successful approaches among communities. The goal is to create opportunities for good work to spread efficiently, and make it easier for successful models to be shared and adapted to local needs. To date, spread networks have been initiated for:
- Adult Mental Health and Substance Use
- Chronic Pain
- Coordinating Complex Care for Older Adults (including Palliative Care)
- Maternity
